- The student must have an illness, condition or disability that causes a substantial limitation of his or her ability to learn, or have a substantial limitation in one of the other major life activities to be eligible for a 504 plan.

Major life activities include:
walking, seeing, hearing, speaking, breathing, learning, working, caring for oneself, and performing manual tasks. - Your school guidance counselor, school nurse or principal will assist you in developing a 504 Plan.
- Students who do not qualify for an IEP or Special Education might be eligible for a 504 Plan.
- A 504 Plan is not a Special Education service.
